Pathophysiological Mechanisms

In early metabolic dysfunction, skeletal muscle and hepatic insulin receptors become resistant to signaling. The pancreatic beta-cells compensate by secreting 3 to 5 times more insulin to force glucose into cells, keeping fasting blood glucose and HbA1c completely normal for years. Only when beta-cell secretory exhaustion occurs does HbA1c cross the 5.7% prediabetes threshold.
