What Is Hemoglobin A1c Biochemically?#

Inside every red blood cell, hemoglobin molecules transport oxygen from your lungs to your peripheral tissues.

When glucose circulates in the bloodstream, it non-enzymatically bonds to the N-terminal valine residue of the hemoglobin beta chain - a chemical process known as the Amadori rearrangement or glycation:

  1. Non-Reversible Bond: Once a hemoglobin molecule is glycated, it remains glycated for the remainder of the red blood cell’s 90 to 120-day lifespan.
  2. Proportional Glycation: The higher your average blood sugar levels, the higher the percentage of your hemoglobin molecules that become sugar-coated (glycated).
  3. Recent Bias: Because red blood cells are constantly being created and destroyed, roughly 50% of your A1c result reflects blood sugar levels over the preceding 30 days, while the remaining 50% reflects the prior 60 to 90 days.

Confounding Factors: When A1c Can Be Falsely High or Low#

Because A1c relies on red blood cell survival, certain conditions alter test accuracy:

  • Falsely Low A1c: Hemolytic anemia, recent blood transfusion, acute blood loss, or enlarged spleen (red blood cells are destroyed prematurely before they have time to accumulate glycation).
  • Falsely High A1c: Iron deficiency anemia, Vitamin B12/Folate deficiency, or splenectomy (red blood cells live longer than normal, accumulating excess sugar coating).
