The Pathophysiology: How Autoantibodies Destroy Thyroid Cells#

1. Hashimoto's Thyroiditis: Cytotoxic Destruction#

  • The Mechanism: CD4+ T helper cells recruit autoreactive CD8+ cytotoxic T-lymphocytes into the thyroid parenchyma.
  • Follicular Apoptosis: High titers of TPOAb and TgAb fix complement proteins to follicular basement membranes, driving progressive cell lysis, chronic fibrosis, and permanent glandular atrophy.

2. Graves' Disease: Continuous Receptor Stimulation#

  • The Mechanism: Instead of destroying cells, TSI autoantibodies act as "fake TSH" agonists, locking TSH receptors into a permanent "ON" state.
  • Thyrotoxicosis: The thyroid gland swells symmetrically (diffuse toxic goiter) and pumps out uncontrolled quantities of thyroid hormones, causing rapid heartbeat, heat intolerance, weight loss, and bulging eyes (Graves' orbitopathy).

Seronegative Autoimmune Thyroiditis: The 10% Exception#
