Healthy bone marrow produces red blood cells that are remarkably uniform in size (each measuring approximately 6 to 8 micrometers in diameter with a Mean Corpuscular Volume / MCV around 85 to 95 fL).

RDW measures the mathematical variation in cell size (anisocytosis):

RDW-CV (%) = (Standard Deviation of RBC Volume / Mean Corpuscular Volume [MCV]) × 100
  • Normal RDW (11.5% to 14.5%): Red blood cells are homogenous and uniform in size.
  • Elevated RDW (> 14.5% to 15.0%): The bloodstream contains an abnormal mixture of very small, normal, and very large red blood cells.
