The Anion Gap is a mathematical calculation performed on your routine Comprehensive Metabolic Panel (CMP) that evaluates your body's acid-base and electrolyte balance. Calculated by subtracting measured negative ions (Chloride and Bicarbonate/CO2) from positive ions (Sodium), the normal range is 4 to 12 mEq/L. An elevated anion gap warns doctors of unmeasured acids in your blood (such as lactic acid, ketones, or kidney waste), while a low gap is most often caused by low albumin protein.
When reviewing a routine Comprehensive Metabolic Panel (CMP) or electrolyte panel, you may notice a calculated result near the bottom labeled "Anion Gap" (or Anion Gap, Serum).
Unlike sodium or glucose, the lab does not directly measure an "anion gap" molecule in your blood. Instead, it is a mathematical formula that evaluates your body's delicate acid-base (pH) balance.

How the Anion Gap Is Calculated#
Your blood contains electrically charged particles called electrolytes:
- Cations (Positively Charged): Primarily Sodium (Na+) and Potassium (K+).
- Anions (Negatively Charged): Primarily Chloride (Cl-) and Bicarbonate / Carbon Dioxide (HCO3- / CO2).
In human physiology, the total number of positive charges always equals the total number of negative charges (electrical neutrality). However, standard blood tests only measure the major electrolytes, leaving some negative ions (like proteins, phosphates, and sulfates) "unmeasured."

What Causes a High Anion Gap (> 12–14 mEq/L)?#
A High Anion Gap indicates that excess unmeasured acids are accumulating in your bloodstream, a condition doctors call Metabolic Acidosis.
Here are the most frequent causes:
1. Lactic Acidosis (Strenuous Exercise or Dehydration)#
When tissues don't receive enough oxygen, cells produce lactic acid.
- Common benign cause: A hard gym workout, marathon run, or severe dehydration right before your blood draw.
- Clinical cause: Poor tissue perfusion during severe infection (sepsis) or heart failure.
2. Ketoacidosis (Diabetic or Starvation/Fasting)#
When your body burns stored fats rapidly without adequate carbohydrate processing, the liver produces ketone acids (acetoacetate and beta-hydroxybutyrate):
- Diabetic Ketoacidosis (DKA): A serious complication in Type 1 or advanced Type 2 diabetes when insulin is severely lacking.
- Extended Fasting: Harmless, mild elevations in ketones from multi-day fasting or a strict ketogenic diet.
3. Kidney Failure / Uremia#
Healthy kidneys excrete organic acids (sulfates and phosphates). When kidney filtration slows significantly (low eGFR / high creatinine), these acids back up in the bloodstream.
4. Toxic Ingestions#
Accidental ingestion of substances like methanol (antifreeze), ethylene glycol, or high-dose aspirin (salicylates).
If you were mildly dehydrated or did an intense workout before your morning fasting blood test, a slightly elevated anion gap (e.g., 13 or 14 mEq/L) is very common. Drinking water and re-testing when fully rested typically brings the number right back into the normal range.

What Causes a Low Anion Gap (< 4 mEq/L)?#
Seeing an anion gap below 4 mEq/L is much less common and is typically driven by:
- Low Blood Albumin (Hypoalbuminemia): Albumin is the primary unmeasured negative protein in your blood. If your albumin is low (due to poor nutrition, liver stress, or inflammation), your calculated anion gap drops naturally.
- Laboratory Measurement Artifact: Minor calibration variances in chloride or sodium test electrodes.
- Elevated Immunoglobulins (Rare): Conditions like Multiple Myeloma that produce large amounts of positively charged antibody proteins (paraproteins).
