Transferrin Saturation Percentage (TSAT) is integral to systemic iron homeostasis, erythropoiesis, cellular respiration, and electron transport chain efficiency. Systemic iron flux is governed by the hepatic peptide hormone hepcidin, which binds and induces degradation of the sole cellular iron exporter ferroportin. Cellular iron uptake occurs via transferrin receptor 1 (TfR1) endocytosis, with storage in high-capacity multimeric ferritin complexes. Pro-inflammatory cytokines (IL-6, TNF-alpha) upregulate hepcidin synthesis, causing macrophage iron trapping and functional anemia of chronic disease.
